Two monitoring templates have been created to help teachers align their teaching and assessments to state standards and to use their classroom data to monitor student progress on those standards. The Individual Student Monitoring Template monitors individual student progress on the standards whereas the Class Monitoring Template monitors a whole class of the students on the standards. Teachers can choose templates that include all of the Maryland content standards in their template or they can choose templates where they choose which indicators and objectives to include. Take a look at the templates that we've designed and customize them to meet your needs.
Individual Student Monitoring Template
The Individual Student Monitoring Templates provides space for all of the objectives assessed on MSA in reading, mathematics and science for grades 3 through 8 and all of the Core Learning Goals and indicators assessed on HSA for high school. You can also choose an individual student monitoring tool for all of reading, mathematics and science indicators and objectives in pre-K to 2 and all of the indicators in high school courses that have an approved voluntary state curriculum.
The individual student monitoring template might be used by a a teacher to monitor progress of a whole class or of a smaller group of underperforming students. It might also be used by students to track their own progress on the objectives they are expected to learn and will be assessed on that year in mathematics and reading. It might be part of a student's reading or math portfolio that would also include the student work aligned to the objectives being monitored.
These templates print in landscape or portrait mode with print headers and footers turned off.

Class Monitoring Template
The class monitoring templates can be used by teachers and the school for a variety of purposes:
These templates are all customized by users selecting from a master checklist of all Maryland content standards which indicators and objectives they want to include in their monitoring template. Teams of teachers could look at their unit of study and identify which Maryland content standard objectives they were teaching as part of that unit. By placing a check beside the objective they will be monitoring during a unit of study or grading period, they can print out a grade book or monitoring plan customized for their use. They can also download the worksheet (by clicking on the download box) into an Excel spreadsheet on their desktop to edit and further customize as they wish. Downloading your data into Excel will also allow teams to collect data across the year, as well as sort and graph the data for additional analyses and more comprehensive monitoring.
These templates print best from Excel or in landscape mode.
